FormATable costs $125 and another $25 for the webmaster edit screen if you want to be able to add, edit, and delete the data from a web page.
Basic installation is included in the price which allows unlimited usage on one website and the product supports either a database such as MySQL or a flat file implementation.
Free upgrades and support are included for the life of the website.
